12 Week Get Your Health Back on Track Programme
How can the 12 week get your health back on track programme help you?
This programme is ideal for anyone with a more chronic health condition, where more intensive support is needed to achieve your health goals. This could include endometriosis, PCOS, irritable-bowel syndrome or auto-immune conditions. The programme provides weekly support on implementing the recommended changes through practical handouts, recipes and meal plans and means that you are not left on your own as you start to make improvements to your health.
Nutrition programmes are the best way of making nutritional therapy work for you and supporting you to achieving your health goals. Instead of being given a nutrition plan and then being left to implement it for four to six weeks, this programme gives you intensive weekly support over a twelve-week period.
This means we can work together to come up with a way of working that suits you. Instead of being given a long and possibly overwhelming list of things to implement in between monthly appointments, we will build on the plan week by week. So, in the first week, we may just work on changing one or two things. We’ll then review how this goes in the second week and either make some adjustments if these changes were difficult and talk about how we could make it easier or if it went well, add in some additional things to change to build on your progress. The weekly support helps to address any questions or challenges you are facing straight away and to refine the plan on an ongoing basis. It is also a good way to keep up your motivation to achieve your health goals. Some people need some accountability to help them make changes – and having a weekly call to talk about your progress with implementing the recommendations is a great way to do this.
How does the 12 week get your health back on track programme work?
You will be sent a food diary and nutrition questionnaire to complete before your appointment, to allow me to do some preparation before your appointment and understand more about your health concerns, current diet and lifestyle. It also gives me chance to do some research on any supplements and medications you are currently taking.
In week one, we will have a 90-minute consultation to discuss your health goals in more detail, your food preference and cooking habits, along with talking about your lifestyle, sleep patterns and ways you relax. We will also talk about what you have tried before, what helps your symptoms and what makes them worse. We will agree a pace of working that suits you and the type of support you would find most helpful so that I can tailor a package of support for you.
We will start with a few recommendations to try in week one. I will also make any recommendations for supplements or tests that may be helpful from your GP or private testing companies.
We will then have calls for the next ten weeks to review progress and build on this. In week twelve, we will have a final review appointment and I will prepare a maintenance nutrition plan for you to use going forward.
If you feel you would like further support at the end of the plan, you can always book follow-up appointments.
